Additional information: ALL REFERRALS TO THIS PROGRAM WILL BE FORWARDED TO THE CENTRAL INTAKE FOR MENTAL HEALTH Bio-psychosocial Rehabilitation emphasizing client choice, empowerment, and individual strengths, is provided on a one-to-one basis. Services include care planning, problem solving, coaching on therapeutic lifestyle choices and education on health and medication. As well, advocacy in accessing financial benefits, housing, food, clothing and community health services is provided. Crisis prevention, consultation and coordination, monitoring, support and life skills are also important functions of the Case Management role. This program is open to clients ages 16 and over whom are willing to actively participate in treatment including working towards client driven goals. Clients appropriate for this programs would have their life being significantly affected by mental health concerns, and significant life stressors that would make focusing on therapy difficult.
