Additional information: Inner City Family Health Team is a primary care organization that provide team based care to homeless and previously homeless individuals in Toronto. We address their health, housing, and social care needs while transitioning to community based living. Our neuropsychologists collaborate with Inner City Health Associate (ICHA) physicians, our interdisciplinary team, and the broader community, hospital and shelter staff to assess individuals with brain injuries, developmental disabilities and/or progressive neurocognitive disorders who are currently homeless or have a recent history of experiencing homelessness. In direct collaboration with referring physicians and nurse practitioners, we help support clients’ disability applications needs as it relates to the Ontario Disability Support Program (ODSP), Developmental Service Ontario (DSO), the Canada Disability Tax Credit (DTC) and Canada Pension Plan Disability (CPP-D) applications.
