Additional information: VON Durham Hospice Services is a not-for-profit organization that serves individuals in Durham Region and their families facing advanced illness, death, grief and bereavement through volunteer based Palliative Care, Caregiver Support, Bereavement Support, Community Awareness, and Fundraising Services. VON Durham offers the following services: - Palliative Care Community Team (PCCT) - along with the individual's Circle of Care, provides holistic person-centred palliative care to individuals from early diagnosis through to bereavement. - Grief & Bereavement - assists individuals who have experienced a death. Our Grief and Bereavement Care Coordinators provide assessment, and service provision for individuals living and coping with grief and bereavement. The Coordinators also provide continuity of care with one-to-one programming, support groups, education, and referrals to other community support services when appropriate. - Caregiver Support (non-PCCT related) - our caregiver supports are specific to those caring for an individual with a life-limiting illness.
