Additional information: Who We Are Our multi-disciplinary team provides geriatric psychiatry consultation and time-limited pharmacological and psychosocial treatments to adults 65 years or older who are experiencing issues with their mental health, including dementia. Psychosocial interventions include caregiver education and support, and collaboration with community partners. Eligibility Criteria • Age 65+ (except for cognitive assessments) • Able to attend outpatient care • Address in Toronto or GTA Exclusions: • Received geriatric psychiatry care in ON within the last year • Referring clinician belongs to a hospital system that includes geriatric psychiatry services • Referral for 3rd party assessments (e.g. child welfare services, WSIB, psycho-educational assessment, medico-legal assessment) Attachments To help us provide the best care, please include all relevant documents including previous psychiatric/ geriatric/ neurologic consultations or discharge summaries, cognitive or psychological reports, medication sheets, medical reports, lab and test results. Please include information about the circle of care (e.g. case manager, SDM or family member assisting with appointments) and accessibility issues (cognitive, language, sensory, or mobility concerns) to support access to care. Additional Information We are not an emergency service. If your patient is too acutely ill to wait for an assessment, please consider accessing a Psychiatric Crisis Service or Emergency Department at the nearest hospital. If a crisis situation arises, please inform them to go to their closest Emergency Department. Patients will be contacted regarding an appointment. Intake staff will make two attempts to reach the patient and leave two voice mail messages. The number will appear as Sinai Health. If we are unable to reach the patient, the referral source will be notified and the referral will be inactivated. Patients are welcome to contact us directly at 416-586-8888 ext. 5192 to discuss their referral at any time. Please note we are a teaching hospital, so care may be provided by medical students, residents or other medical trainees.
