Additional information: The Nurse Practitioner (NP) embedded with Renfrew County Paramedic Service provides accessible, community-based primary and urgent care to support patients who are unattached to a primary care provider, unable to access their usual provider, or experiencing barriers to timely care. Referrals are appropriate for assessment and management of acute or chronic medical concerns, diagnostic investigation and interpretation, medication initiation or adjustment, management of complex or unsupported care needs, and coordination of referrals to specialty or community services. The NP also supports patients who may otherwise require Emergency Department assessment when their needs can be safely managed in the community, as well as individuals connected to Community Paramedicine, palliative care, homelessness/addictions services, and other community-based programs. The service is intended to complement-not replace-existing primary care, with referrals particularly appropriate when a patient has no primary care provider, their provider is unavailable, or additional clinical assessment, diagnostic work-up, treatment, or care coordination is required.
