To provide comprehensive care, your patient may be referred to one or more of these geriatric services: Behavioural Supports Ontario (BSO) Program, which operates out of the Mental Health Department at STEGH and provides consultation for older adults experiencing responsive behaviours associated with cognitive impairments, mental health or addictions, and includes geriatric psychiatry (community based; assesses persons in their home, long term care, or hospital; accepts referrals from any source); a Memory Clinic led by a Geriatric Psychiatrist, whose team includes nursing, social work and staff from the local Alzheimer Society, and which requires a Physician or Nurse Practitioner referral; Alzheimer Society Southwest Partners (self-referral, 519-633-4396); and a Geriatric Resource Nurse who provides home visits, consults with a Geriatrician, and requires a physician referral.
