Additional information: CONNECT is a centralized intake service for adult outpatient mental health and addictions programs at St. Joseph’s Healthcare Hamilton. Our service receives and reviews referrals, completes preliminary screenings, and matches clients with the most appropriate, evidence-based supports based on their care needs. Referrals can be made by any healthcare provider or community agency, and clients can also self-refer through the CONNECT website at www.stjoes.ca/connectmhap.
