SGS Central Intake is the single point of entry to SGS programs across the Champlain region. SGS programs assess, diagnose, and treat older adults experiencing issues such as functional decline, cognitive impairment, falls, polypharmacy, behavioural or psychological symptoms, and caregiver strain. Services are delivered across the continuum of care by interdisciplinary teams with expertise in geriatric medicine, geriatric psychiatry, seniors' mental health, care of the elderly primary care, and interprofessional geriatric teams.
