Not accepted: GMHOT provides short-term care only. Other programs at this intake have their own age limits: Clinical Case Management 16+, RAIT 26-64 and only for patients of a primary care provider actively linked to RAIT, LAI Clinic only for clients with a current psychiatrist prescription for an injectable psychotropic or clozapine monitoring, TNT early intervention 14-35, Mental Health First Response 16+.

Note: All referrals to the below listed programs will be coordinated through the Chatham-Kent Mental Health & Addictions Central Intake for CMHA & CKHA. Address: 240 Grand Avenue West, Chatham ON, N7L 1C3 Mental Health & Addictions Program (CKHA): MHAP provides a variety of Mental Health & Addictions support in Chatham-Kent including the following programs: Mental Health Counseling and Treatment, Psychogeriatric Mental Health Outreach Team (GMHOT), Addictions Assessment, Treatment and Referral Program, Concurrent Disorders Program, Problem Gambling and Eating Disorders, Assertive Community Treatment Program. Geriatric Mental Health Outreach Team (GMHOT): A multi-disciplinary team providing short-term care for individuals over 65 years of age experiencing a mental health issue. The nurses, nurse practitioner, social worker, occupational therapist and psychiatrist work with client, family and caregivers to improve quality of life. Services available include assessment, recommendations, medication review and adjustments, therapy, and referral to other agencies as required for those living in their homes, retirement homes, group homes or long-term care homes. Referral from a nurse practitioner or physician is preferred, though not required. Clinical Case Management: Bio-psychosocial Rehabilitation emphasizing client choice, empowerment, and individual strengths, provided one-to-one. Open to clients ages 16 and over willing to actively participate in treatment. Rapid Assessment Intervention Treatment (Referred by RAIT physician): short-term mental health services in partnership with Primary Health Care Practitioners across Chatham Kent; may include recommendations regarding need for psychiatric consultation, nursing assessment and treatment, psychotherapy treatment, and psychosocial interventions. Open to clients ages 26-64 who are currently a patient of a Primary Care Provider actively linked to the RAIT program. Long Acting Injection (LAI) Clinic: Administration of psychiatric medication, assessment and monitoring of individuals receiving injections, and metabolic syndrome monitoring. Open to clients with a current prescription from a psychiatrist for an injectable psychotropic medication, or prescribed clozapine and requiring monitoring. Mental Health First Response Team (Including Addiction): Monday to Friday 8:30-5 in person, and by phone at 1-866-299-7447 24/7. Telephone crisis intervention, referrals, and family support. Open to clients ages 16 and up. Today Not Tomorrow (TNT) Early Intervention for psychosis: clients age 14-35 experiencing first episode psychosis or prodromal symptoms. Housing: support and outreach for clients experiencing mental health symptoms who need emergency or stable housing; clients should not already be involved with housing supports from another agency.
