Who they see: Inclusion criteria: 65+; lives at home or in a retirement home; must be a client of one of the named Primary Care Providers with MoUs. Geriatric issues covered: cognitive loss, functional decline, diagnosis and medication review, driving safety, family/caregiver concerns, falls/mobility, home safety, failure to thrive/frailty, depression with additional medical problems, frequent ER visits or hospital admissions.
Not accepted: Exclusion criteria: patients under 65 years; behaviours; delusions/hallucinations; psychiatric diagnosis; dual diagnosis. All referrals are automatically routed to SGS Central Intake, the single point of entry to SGS programs across the Champlain region.
