Not accepted: • Assessment by a psychiatrist within the last 12 months • Followed by a psychiatrist or followed by a psychiatrist in the last 12 months • Referring MD/NP from a clinic/hospital affiliated with hospital with a department of psychiatry • Referral for 3rd party assessments (e.g. child welfare services, WSIB, psycho-educational assessment, medico-legal assessment) Attachments PHQ9 and GAD7 for mood and anxiety assessments. To help us provide the best care, please include all relevant documents including previous psychiatric/ neurologic consultations or discharge summaries, psychological reports, medication sheets, medical reports, lab and test results. Additional information We are not an emergency service. If your patient is too acutely ill to wait for an assessment, please consider accessing a Psychiatric Crisis Service or Emergency Department at the nearest hospital. If a crisis situation arises, please inform them to go to their closest Emergency Department. Additional information: Who We Are The General Psychiatry Clinic accepts referrals from physicians and nurse practitioners for adults aged 18-64 who require diagnostic assessment and treatment planning. Patients are seen for a comprehensive psychiatric assessment. Following this assessment, a written report with recommendations is sent to the referring provider (and Primary Care provider if that is not the referring provider). The treatment plan may include information on: • Medication options • Types of psychotherapy likely to be helpful • Education and self-directed resources • Specialized hospital-based or community programs outside of our department In some cases, the plan may include a time-limited period of follow-up psychiatric care, referred to as an episode of care. This may include short-term services within our department, such as medication initiation, psychotherapy, skills-based groups, or support with connecting to community resources.
