Eligibility (confirm before referral) * Patient ≥ 65 years * Resident of Southwestern Ontario (LHIN 1 or LHIN 2) * Living in the community or retirement home (not currently in hospital or long term care) *Substitute Decision Maker and/or Power of Attorney available to attend appointment Reason for Referral (1)Cognitive / Functional * Suspected dementia or cognitive decline * Functional decline (ADLs/IADLs) * Delirium or fluctuating cognition * Falls, wandering, or driving concerns (2) Mood / Behaviour * Depression or anxiety * Behavioural symptoms of dementia (agitation, aggression) * Suicidal ideation * Substance use concerns (3) Complex Care * Polypharmacy or medication review * Caregiver stress * Self neglect, unsafe living conditions, or elder abuse concerns * Other geriatric mental health concern
