Additional information: Please indicate reason for referral –Must be 18 years of age or older and meet at least one of these criteria: • Gastric mass identified at time of endoscopy • DI report suggestive of gastric mass • biopsy proven result of gastrointestinal stromal tumor (GIST) • biopsy proven gastric neuroendocrine tumor • biopsy proven gastric cancer adenocarcinoma The gastric DAP clinic will complete all staging investigations. The clinic coordinates all diagnostic and staging investigations. Our clinics are very busy and therefore your visit can take between one to three hours to complete. Many factors make the timing of your visit unpredictable, including the number of patients we are serving in the clinic, the wait for additional testing, and procedures being more complicated than initially anticipated. This recipient currently receives referrals from the referral network via eDT*, as part of eReferral Ontario. As such, updates on the status of the referral (or other updates) will not flow back through the referral network, and any follow-up requests should be sent via other means. Further, if you need to cancel or modify this referral, you will have to cancel or modify the referral in the referral network, AND notify the recipient provider directly.
