Additional information: Community Paramedics provide services that help patients with complex health conditions continue to live independently in their own homes, while helping to reduce unnecessary emergency department visits and hospital admissions. They provide in-home clinical assessments and disease process monitoring, heath education, point of care diagnostics, and treatment modalities extending beyond the roles of traditional emergency care and transport. Community paramedicine clients acquire the tools to become more independent in the self management of their conditions, including making the right call to the right place at the right time when their own ability to manage is overwhelmed.
