Additional information: Mobile program serving Renfrew County 3/week in Pembroke and 2/week in Renfrew Interdisciplinary team including Care of Elderly (CoE) physician (Pembroke site) and Geriatrician (Renfrew site), in addition to RN, OT, PT and SW support. Goal is to keep patients in the community as long as feasible by setting individual goals regarding function. Some of the reasons patients are referred to the Geriatric Day Hospital include: Multi-complexity (multiple chronic conditions, advanced illness, complicated social needs) Mind (cognition, driving safety and mood) Mobility (balance, strength, function, fall prevention) Medications (multiple medications, de-prescribing, adverse medications effects) What Matters Most (Patient's own meaningful goals and preferences) GDH has weekly class in both sites that include PT led exercise class, education, and brain fitness Home visits as necessary Please note that all referrals to our program will be automatically routed to SGS Central Intake, which is the single point of entry to SGS programs across the Champlain region.
