Additional information: The Freeman Outreach Program enables adults living with advanced cancer, whose wish is to stay at home for as long as possible or to die at home, to receive in-home care from a Freeman outreach physician with the support of other community supports as needed. Eligibility Criteria: Diagnosed with Advanced Cancer diagnosis; Homebound (< PPS 50%), cannot leave their homes, and find it difficult to attend clinic appointments; Looking to improve their quality of life through a palliative care approach that focuses on managing pain and other physical and/or emotional symptoms, with no ongoing cancer treatment; Living within the North York catchment area: South of Steeles Ave., West of Victoria Park, North of Lawrence Ave., and East of Bathurst St. Please include the following when referring your patient: Palliative Care Common Referral Form; Medication List; Latest Clinical Note.
