Who they see: Patient receiving care from a Mount Sinai OB/FHT/midwife or referred by MSH staff (ED, 9S, Wasser Pain Clinic); preconception, pregnant or within 1 year postpartum; strong history of mental illness or active symptoms (mood/anxiety, psychosis, inability to care for self/baby); 18+
Not accepted: Not pregnant or >1 year postpartum; normal grief; under 18; if patient has an active psychiatrist the referral must come from them; substance use or ADHD as primary reason; 3rd-party assessments (child welfare, WSIB, psycho-educational, medico-legal)
What to include: Psychiatrist notes if any; previous psychiatric/neurologic consults, discharge summaries, psychological reports, medication sheets, medical reports, labs
