Additional information: The professionals in our specialized geriatric clinics, geriatric day hospital and outreach teams conduct comprehensive geriatric assessments to reduce the burden of disability by detecting and treating reversible conditions, management of chronic conditions and optimizing function. The team works closely with partners and service agencies available in our community in an effort to assist patients, families and care providers to access the care they need. We work in partnership with regional networks including Home and Community Support Services, primary care providers, health care professionals, multicultural organizations and others dedicated to the health and well-being of older adults. Additionally, our team works closely with the Regional Geriatric Program of Toronto, who assists in providing system level coordination of senior friendly services, capacity building and to help advance the quality of care.
