Additional information: St. Joseph's Continuing Care Centre (SJCCC) offers programs targeting the senior population of Eastern Ontario and the Great River OHT. SJCCC provides inpatient rehabilitation services to adults who require reconditioning and/or retraining following illness, injury, surgery, or a decline in health and mobility. The inpatient program can have a length of stay from as short as two weeks and up to 90 days. SJCCC also offers a "Hospital to Home" Remote Care Monitoring (RCM) Program following transitions of adult and geriatric patients to ensure care plan adherence and a smooth handoff to other community supports. Patients receive remote "check-ins", video calls, and secure messages to and from a Remote Care Nurse to ensure they are adhering to their care plans, along with health teaching and vitals capture via Bluetooth devices in an internet-connected health kit. Palliative Care is also available at SJCCC. Our multidisciplinary team of trained professionals works together to provide the best end-of-life care possible by providing care to both the individual and their loved ones during this final stage.
