Who they see: Patients aged 18+ who are medically complex, have unmanaged chronic conditions, or struggle with managing their health due to mental health, cognition, or social determinants of health concerns. Partners with family physicians in solo practice. As long as at least one of patient or physician is within the North Toronto (NT) sub region, referral will be considered.
Not accepted: The patient is under 18 years of age. Patient already enrolled with an interprofessional team. Both patient and primary care physician are outside of the NT sub region catchment area and/or not connected to IPCT through other projects.

Additional information: The North Toronto Interprofessional Primary Care Team (IPCT) at Baycrest partners with family physicians in solo practice to provide interprofessional primary care for patients aged 18+ who are medically complex, have unmanaged chronic conditions, or struggle with managing their health due to mental health, cognition, or social determinants of health concerns. The goal is to enhance patient outcomes and well-being through coordinated care. The team includes NP, OT, PT, SW, RD, Pharmacist, Mental Health Case Manager, HCCSS Care Coordinator, Dementia Navigator and Digital Health support. Exclusion criteria: - The patient is under 18 years of age - Patient already enrolled with an interprofessional team - Both patient and Primary care physician are outside of NT sub region catchment area and/or not connected to IPCT through other projects *Please note as long as at least one (Patient or Physician) are within NT region referral will be considered
