Additional information: The Mint Memory Clinic is an interdisciplinary approach to supporting primary care providers through a comprehensive assessment and care plan development for patients experiencing memory concerns and their caregivers. Through this clinic, patients and their caregivers are supported by a specially trained physician, registered practical nurse, occupational therapist and The Alzheimer's Society of Lanark Leeds and Grenville. Note: Referrals from Lanark County beyond Perth and Smiths Falls will be considered if wait times allow. The Mint Memory Clinic is conducted in 2 separate appointments: 1. In home (preferrable) visit (approx. 1-2 hours) to conduct memory testing and risk assessment with RPN and OT. Please note this can also be conducted in clinic. 2. Office appointment (approx. 1 hour) conducted by physician, RPN and meet with the Alzheimer's Society. In addition to a referral, patients are required to have the following diagnostic tests completed prior to their referral: 1. Blood Work (CBC, Na, K, Cl, Cr, Glucose, HbA1c, Calcium, TSH, B12) 2. ECG 3.
