Additional information: Adults 65 years of age and older experiencing mood, personality or behavioural changes, who are in need of psychiatric evaluation and consultation. Please note that all referrals to our program will be automatically routed to SGS Central Intake, which is the single point of entry to SGS programs across the Champlain region. The triage process ensures referrals are directed to the most appropriate service. Calculated Wait Time 1 Unspecified Wait Time 2 Unspecified Wait times will vary within the program based on geographical location of the client Comprehensive psychiatric assessment, in-home or in long term care settings. Pharmaceutical behavioural and psychosocial consultation. Liaison with family physicians, professional caregivers, community agencies and family. Advocacy Geriatric mental health education and behaviour management coaching of professional care providers. BSO: Support is provided to LTCHs GMH RN provides additional coaching for individual client's behaviours to LTCH staff upon request and can refer clients to Behaviour Therapist for assessment and Behaviour Care Plan. Community Transition Behaviour Support, is provided by referral to clients qualifying for GMH PRC: Education provided upon request and / or annual plan
