Additional information: The Slaight Brain Health Program is a partnership between Sunnybrook Health Sciences Centre and SPRINT Senior Care with a mission to support tenants in the following four Toronto Seniors Housing buildings: 71 Merton St., 384 Mt Pleasant Rd, 801 Mt. Pleasant Rd., 130 Eglinton Ave E. The aim of this program is to bring wrap around support to tenants where they live to enable them to optimize their brain health and live in their homes for as long as possible. With a thorough understanding of the social determinants of health and health equity, our program supports tenants in both reducing the risk of and delaying the onset of dementia, and of coping with cognitive impairment and dementia. Our team members include a Recreation Therapy Supervisor, Activationists who run brain health programming, an Occupational Therapist, and a Nurse. We operate within the model of the Neighbourhood Care Team. The Neighbourhood Care Team (NCT) is an integrated geriatric model of care that unites dedicated health care organizations, providers, and their clients (patients, families, and caregivers), to improve the integration and coordination of care as one team to improve health outcomes and support aging at home. The model is tenant driven and comprises approximately 15 organizations working together as one team. The Slaight Brain Health Team can connect tenants to members of their NCT as needed. Sunnybrook Health Sciences Centre is located in Toronto, Ontario and is one of Canada's leading Academic Health Sciences Centres. With 1.1 million patient visits every year, our mission is to care for patients and families when it matters most. Sunnybrook is home to Canada's largest Trauma Centre. SPRINT Senior Care is a Community Support Service agency committed to providing exceptional care and quality services that enable seniors to live at home and thrive in their communities. We began caring for seniors and enabling seniors to care for themselves in 1983. We continue to do so today as an accredited, not-for-profit community support service agency in Toronto by offering a wide range of practical and low-cost services to seniors and their caregivers, including home care, adult day programs, transportation, Meals on Wheels, caregiver support, and health and wellness activities and initiatives.
