Additional information: Our services are covered by OHIP( except for therapy) “Dr. Amer is participating in Triaging Referrals as eConsult and your eReferral may be responded to as an eConsult if clinically appropriate.” Peacock Clinic Referral Information Sheet Introduction: The Peacock Clinic is a fully remote-led mental health team offering general psychiatric assessments through a short-term consultation model. Our interdisciplinary team includes two psychiatrists, an addiction specialist, an intake coordinator, and a therapist. We provide diagnostic assessments and short-term medication management, typically involving one to four sessions. Our care-sharing model involves creating a treatment plan that the patient's family doctor will implement. Due to our extensive referral base, we focus on diagnostic assessments and brief treatment management, with ongoing care provided by the primary care provider We utilise a hybrid approach of phone and video for assessments. All initial psychiatric consultations are conducted through video to ensure a comprehensive evaluation. The follow ups can be completed over phone or video.We use various digital tools such as AI scribes ,secure video platforms , the referral network e- referrals, emails and texts to provide optimum Additional Information: - It is essential to include the patient's email, as much of our communication occurs via email and text.- If a patient fails to confirm the appointment or complete the questionnaire without a reasonable excuse, we may need to reschedule or discharge them from our care. - Patients will receive appointment details through the CORTICO platform and must confirm their appointments within 10 days of receiving their appointment package.- Encourage patients to complete all questionnaires included in their appointment package, which may take 20 to 30 minutes, as this aids in the assessment process. 1 to 2 weeks before their appointment, patients will receive a phone call and message from our intake coordinator to schedule a phone intake. - We do not provide services to individuals under 18 years old or those with severe neurodevelopment disorders, neurocognitive disorders, or requiring acute psychosis assessments. - Our typical working hours are Monday-Thursday 9 am to 3 pm. No urgent/emergency/after hours care available. Referral Submission: Referrals can be submitted via the the referral network network, OSCAR, or fax using the provided form. Referrals are triaged within 3 to 4 weeks, and we are currently booking 5 to 6 months from the time of referral. Please inform patients that we strive to accommodate them promptly and will advance appointments if cancellations occur, although this is not guaranteed.
