Additional information: The University of Ottawa Heart Institute (UOHI) implemented a new and innovative Cardiac Supportive and Palliative Care Program which is a specialized outpatient clinic for patients diagnosed with advanced cardiac disease and their caregivers across Eastern Ontario. In working collaboratively with other UOHI programs and community resources, this program focuses on symptom management, self-care strategies, caregiver support and goals of care discussions. Patients with an end-stage cardiac disease are eligible for this program.
