Additional information: This is a 30 minute phone appointment booked with a Social Worker to register clients for our education, social/recreational programs or counselling support (individual/family/group). The Alzheimer Society Waterloo Wellington provides for the Person Living with Dementia and/or their Care Partner: - System Navigation - Education Sessions and Resources - Individual/Family Counselling/Support with Registered Social Workers - Therapeutic/Support Groups with Registered Social Workers - Social/Recreation Programs (including Minds in Motion) To reduce your client's wait-time, please share the New Client Registration form with your client at the time of the referral.
