Additional information: Using the MINT (Multispecialty INterprofessional Team) model, the Burlington Family Health Team Memory Clinic supports people experiencing new or ongoing memory concerns. Our team of specially trained clinicians is made up of a Nurse Practitioner, Nurses, Occupational Therapists, a Pharmacist, a System Navigator, Dr. Jocelyn Anderson, (specialized in care of the elderly), Dr. Junghyun Sarah Park, (consulting Geriatrician) and the Alzheimer’s Society. We will work closely with you in a primary care environment to provide assessment, diagnosis, a treatment plan and support for patients with mild to moderate cognitive impairment and their support person.
