What is a MINT Memory Clinic? MINT stands for Multi-specialty INterprofessional Team. In MINT Memory Clinics, each health care professional has received standardized accredited training to deliver high-quality dementia care. Patients with memory concerns are referred by their family physician to a MINT Memory Clinic for comprehensive assessment and care planning. The MINT Memory Clinic provides a detailed and proactive care plan to the family physician, and refers to specialists when necessary. Our Nurse Practitioner (NP) alongside our patient navigation nurse, pharmacist and administrative team work closely with the family doctor, community agencies and specialist physicians to provide compassionate, person-centered care for the patient and family members.
