For urgent imaging: complete this referral and contact the relevant Diagnostic Imaging department directly per your current practice. Note: Exam prioritization is determined by the Diagnostic Imaging facility based on indication. By submitting a referral through this listing, you acknowledge and agree to the following: Requirement to Obtain Consent This referral is submitted through the Toronto Region Central Intake Hub, operated by University Health Network (UHN), for processing and routing to participating sites. Prior to submitting a referral through the Hub, you are responsible for: Notifying patients that their PHI will be accessed by UHN for the purpose of processing the referral, and that UHN will subsequently disclose PHI related to the referral to a receiving site. Obtaining and documenting patient consent in accordance with applicable privacy legislation. In its role as the TR Central Intake Hub, UHN relies on referring providers to obtain patient consent for this referral process. Please note: This recipient currently receives referrals from the referral network via eDT*, as part of eReferral Ontario. As such, updates on the status of the referral (or other updates) will not flow back through the referral network, and any follow-up requests should be sent via other means. Further, if you need to cancel or modify this referral, you will have to cancel or modify the referral in the referral network, and notify the recipient provider directly. (The recipient provider will not receive the cancellation or modification automatically via eDT*). *eDT (electronic Data Transmittal), similar to eFax Referrals are governed by agreements between OH and UHN, and OH and participating sites.
