Additional information: The Edwin S.H. Leong Geriatric Supportive Care Outreach Program provides supportive and palliative care at home for those living with progressive non-cancer illness. Our program enables adults living with advanced non-cancer life-limiting illness, whose wish is to stay at home for as long as possible or to die at home to receive in-home care. Eligibility Criteria: Living within the North York catchment area: South of Steeles Ave., West of Victoria Park, North of Lawrence Ave., and East of Bathurst St.; Age 60 or above; Homebound (< PPS 50%); Diagnosed with life-limiting progressive non-malignant disease(s); Complex symptoms relating to end-of-life illness that cannot be managed by current care team; Challenges with goals of care discussions/care planning, and decision making about withholding or withdrawing medical treatments; On nursing service with Home and Community Care Support Services and must have adequate support to be maintained at home.
