Medication Refill Request — Ontario Virtual Care

Request a renewal of a prescription you are already on, reviewed properly rather than rubber-stamped.

Running out of a medication you depend on is stressful, and booking a full appointment just to continue something that is working can feel like a waste of everyone's time. This request collects what a prescriber needs to decide whether continuing is still the right thing: what you take, how it is going, and what has changed since it was last reviewed.

Quick Self-Check

Answer a few quick questions. This isn't a diagnosis — a real clinical assessment is the next step either way.

How We Work

The Care365 clinical team reviews what you are taking, why, and how it has gone, then decides whether a renewal is appropriate — and for how long. Where monitoring is overdue or the medication is no longer the right fit, you will be told what is needed instead of simply being refused.

Complete a short online assessment

Answer real, condition-specific questions — takes a few minutes, no appointment needed to start.

The clinical team reviews your answers

A member of the Care365 clinical team reviews your case, typically the same day — not an automated approval.

What Our Clinical Team Can Assess

What you are taking

Medication name, dose, how long you have been on it and how much you have left.

How it is going

Whether it is working, any side effects, and anything that has changed since it was prescribed.

Monitoring where it is needed

Some medications need bloodwork, a blood pressure reading or a review interval before another supply is appropriate.

A real decision

A renewal is reviewed against your history. It is not guaranteed, and sometimes the right answer is a change rather than a repeat.

Common Symptoms

  • A repeat prescription that has run out or is about to
  • A medication that is working and you want to continue
  • A prescription from a clinician you can no longer easily reach
  • Side effects or questions you want looked at before continuing
  • A medication you are not sure you still need

Treatment Options

These options may be prescribed after a real clinical assessment — this is general information, not a recommendation to use any specific medication without one.

Non-medication options

  • Medication review for side effects or effectiveness
  • Monitoring or bloodwork before further supply — Where the medication requires it.
  • Referral back to the original prescriber — For medications outside the scope of an online renewal.

Everything in One Place

  • Easy online scheduling — submit your assessment in minutes
  • Real clinical review, not an automated approval
  • Prescriptions sent directly to your preferred pharmacy when appropriate
  • 100% virtual — no travel, no waiting room

Frequently Asked Questions

Is a renewal guaranteed?
No. A renewal is a clinical decision. Some medications need monitoring, a review interval or a change of plan before more can be prescribed.
Which medications cannot be renewed this way?
Controlled substances and medications needing in-person assessment or recent bloodwork generally cannot. The review will tell you what is needed.
What should I have ready?
The medication name and strength, your dose, how long you have taken it, how much is left, your pharmacy, and any side effects or recent changes in your health.
What if I have already run out?
Say so in the request. If a gap in treatment would be unsafe, contact your pharmacy or a walk-in clinic as well rather than waiting for an online reply.
